How to Apply
To be considered, all organizations must complete our application form. While not required, we will considerand strongly encouragethe inclusion of materials beyond those required in the application that demonstrate how the project meets the mission and funding priorities of QHF. If you would like us to confirm receipt of your application, please send a self-addressed, stamped envelope with your application.
Application
Deadline
Applications will be accepted from 12/3/07 until 1/15/08. All applications
will be reviewed and applicants will be selected in early Spring. Grants
will be awarded in early Summer.
Grant applications and four (4) copies of your entire proposal should be mailed to the QHF staff at the following address:
