Frequently Asked Questions (FAQs)

Q: Do I have to represent a charitable or non-profit organization?

A: Not necessarily. Organizations that are eligible to apply for funding include faith-based, non-profits, government agencies, health care providers, educational institutions, and individuals.

Q: Do I have to be part of a health care organization?

A: Not necessarily. Organizations that are eligible to apply for funding include faith-based, non-profits, government agencies, health care providers, educational institutions, and individuals.

Q: My organization is located in another state, would I be eligible for funding?

A: QHF will support projects in states that are part of Delmarva Foundation's service area (Maryland and the District of Columbia).

Q: What do you mean by an "outcome based improvement project"?

A: An outcome based improvement project is a project that contains outcome based objectives. Outcome based objectives consist of two parts: 1) The statement of what will be achieved, and 2) The method of evaluation and the degree of measurable completion to determine successful completion of the objective. Please see the following example:

Objective: To increase patient knowledge of diabetes self-management and affect behavior change for diabetes self-management as measured by laboratory and clinical measurements in the underserved Central and South American residents of the targeted service area.

Evaluation: Objective outcome trending with significant improvement in measurements such as hemoglobin A1c, FSBS and pill counts (for compliance assessment) and other pre-test/post-test surveys to assess knowledge of participating diabetes patients. Upon post-test patients, knowledge of diabetes and self-management will increase by 80%. Upon completion, conduct an interview with health care providers serving the participating patients to assess perception of patient compliance to treatment recommendations. Of the participating patients, 90% will follow the care provider's treatment recommendations.

Q: What is the maximum number of years a grant can be awarded?

A: Grants are awarded annually, for a one-year timeframe. Projects may apply each year for continued funding.

Q: What is the maximum amount of a grant?

A: $50,000

Q: What kinds of budget items can be requested?

A: Items that may be requested include staff time to support a project or specific marketing materials related to a project. QHF will not fund reimbursable direct patient care services, support of individuals, ongoing operating expenses, capital expenses, lobbying, fundraising activities, or conferences (unless associated with an approved grant).

Q: Do I need letters of commitment from grant collaborators or partners?

A: Yes.

Q: If approved for funding, how often do I have to report project activities and what must be included in the report?

A: If approved, applicants are expected to provide quarterly progress reports in a format to be provided by QHF. These reports should include progress against the timelines submitted, the amount of money spent, the activities conducted, the number of individuals impacted by the activities, and any lessons learned or barriers encountered. At the conclusion of the project and funding period (once per year), the applicant is expected to provide a final report on the project.
