How To Apply
The next grant year will begin on July 1, 2011.
To be considered, all organizations must complete our application form [pdf]. While not required, we will consider—and strongly encourage—the inclusion of materials beyond those required in the application that demonstrate how the project meets the mission and funding priorities of QHF.
If you would like us to confirm receipt of your application, please send a self-addressed, stamped envelope with your application.
Application Deadline
Applications for the 2010-2011 grant cycle will be accepted (tentively) beginning December 1, 2010 and ending January 19, 2011. The final decisions are made by the QHF Board. All applications will be reviewed and applicants will be selected in April/May 2011 and notified of the disposition of their request by early-June 2011.
Five (5) copies of your grant application and of your entire proposal should be mailed to the QHF staff at the following address:
