The Quarterly Report form is purposed to report your quarterly progress to QHF. In the top box that says "Quarterly Report", please note the quarter that is covered in the report. The reports should be complete and should highlight accomplishments made on your milestones, identify any barriers to performance, and detail how the funds have been spent during the reporting period. Attachments may be provided to highlight your activities.
To be considered, all organizations must complete our application form. While not required, we will consider—and strongly encourage—the inclusion of materials beyond those required in the application that demonstrate how the project meets the mission and funding priorities of QHF. (more...)
